Revamped packing slip

We just updated the packing slip, and we really really really really hope you download this, and replace the one you had before!


One of the added features on this will really help us out - we're trying to keep up with our database, and if your snail mail or email address has changed, we want you to check the little box.

Thanks for completely filling out the form for us....it keeps us from having to set your box aside to count cards, or googling the postmark to see if we can guess who it was.
